التُنَيْنة المَدِينيّة بالإنگليزية: Dracunculus medinensis هوطفيلي من الديدان الأسطوانية وهوالمسبب لداء التنينات.
- سميت على اسم المدينة المنورة حيث اكتشفت الكثير من الحالات فيها ، تنتشر في آسيا وإفريقيا (الهند - الباكستان) وغرب آسيا(شبه الجزيرة العربية) وإفريقيا وأمريكا اللاتينية (لا توجد في سورية).
- تتوضع الخيطية تحت الأدمة الجلدية بعد سنة حتى تصبح Adult وهي تتوضع من الخاصرة وحتى الكعب ( فهي من أطول الخيطيات ) وإذا كانت الإصابة شديدة قد تتواجد أكثر من 100 خيطية.
- طولها 1 م _ 1,5 م حسب طول الشخص وقطرها 2 ملم.
- الثوي الوسط هي البلاعط Cyclops .
Dracunculus medinensis is a nematode that causes Dracunculiasis. Dracunculiasis, also known as Guinea Worm disease, is caused by the large female nematode, Dracunculus medinensis, which is the longest nematode infecting humans. The adult female is primarily larger than the adult male. The longest adult female recorded was 800 mm in length, while the adult male was only 40 mm. Mature female worms migrate along subcutaneous tissues to reach the skin below the knee, forming a painful ulcerating blister. They can also emerge from other parts of the body like the head, torso, upper extremities, buttocks, and genitalia. Dracunculus medinensis is one of four filarial nematodes that cause Subcutaneous Filariasis in humans. The other three filarial nematodes are Loa loa (the African eye worm), Mansonella streptocerca, and Onchocerca volvulus (river blindness).
دورة الحياة
- نفس دورة حياة السابقة لكن بملامسة المياه تنفتح الخيطيات وتخترق الجلد وتلقي إلى الوسط الخارجي باليرقات.
- والعدوى تصل للإنسان عن طريق استهلك المياه الملوثة حيث تنحل البلاعط وتخترق لتصل إلى الدوران ثم تتوضع تحت الجلد.
- إذاً اليرقات هنا متواجدة في الوسط الخارجي (الماء) فقط.
فهم الأمراض
Female worms elicit allergic reactions during blister formation as they migrate to the skin, causing an intense burning pain. Such allergic reactions produce rashes, nausea, diarrhea, dizziness, and localized edema. Upon rupture of the blister, allergic reactions subside but skin ulcers form, through which the worm can protrude. Only when the worm is removed, is healing complete. Death of adult worms in joints can lead to arthritis and paralysis in the spinal cord.

الأعراض
- التهابات معممة على طول توضع الخيطيات، حكة جلدية، مفرزات تحسسية، وبثور وتقيح في مكان انفتاح الخيطيات ورائحة كريهة ، وبالإضافة إلى الإقياء والغثيان والإسهال وشرى معمم ، والتهابات مفصلية، ويمكن حتى تهاجر إلى الوجه واللسان والرقبة والأكتاف والأعضاء التناسلية الذكرية لكن هذه التوضعات نادرة.
- وتتحول إلى إزمان أي تعقيدات للحالة السقمية بسبب عدم المعالجة.
المعالجة
دي إيتيل كاربامازين .
- Niridazole 25 ملغ/ كغ يعطى لأسبوع(Ambilhar)فمويا .
- Mintezole 40 _ 50 ملغ/ كغ لأسبوع .
- عندما تتواجد بكثرة تحت الجلد يجب التدخل الجراحي.
- وتسحب باللف على عود حيث يسحب جميع يوم عدة سنتمترات حتى لا تنبتر حيث بحاجة إلى جراحة عندئذ .

Ghana is winning the battle against the guinea worm disease, recording a zero infection rate in the month of November 2009. In the year 2008, 501 new cases were recorded, against 3,357 cases in 2007, representing about an 85 percent reduction. A number of interventions in the past 2 years have been pursued, including monitoring dams and other sources of drinking water in endemic communities to ensure that infected people did not contaminate the water sources. Additionally, filters and other water treatment equipment have been provided at both water pumping sites and homes to treat water before drinking. This is all due to the efforts of the National Guinea Worm Eradication Program. The program is funded by the World Health Organisation (WHO), UNICEF, Japanese International Cooperation Agency (JICA), and European Union (EU).
